COMPARISON OF THE SENSITIVITY OF THE POLIMERASE CHAIN REACTION FOR DETECTION OF CHLAMYDIA PNEUMONIAE INFECTIONS AFTER TWO DIFFERENT APPLIED METHODS FOR DNA EXTRACTION FROM SPUTA, TRACHEAL AND BRONCHIAL SECRETS

SUMMARY: Chlamydia pneumoniae (Chlamydophila pneumoniae) is one of the important pathogens in human respiratory tract infections. The microogranism is an obligatory intracellular gram-negative bacterium that is common cause of bronchitis, sinusitis, pharyngitis and atypical pneumonia. 90% or more of C. pneumoniae infections remain asymptomatic. There is suggested a relationship between C. pneumoniae and chronic diseases such as chronic obstructive pulmonary disease, coronary heart disease, multiple sclerosis, reactive arthritis, Guillain-Barre’ syndrome, Alzheimer’s disease, etc. The knowledge for the epidemiology of C. pneumoniae infections mainly depends on serological detection received from studies of the population immune status using microimmunofluorescence and cell culture. These methods are time consuming and not very sensitive. Recently the isolation of the microorganism was improved and more frequently the PCR started to be applied as a method of choice to diagnose C. pneumoniae infections. The method is performed within several hours and gives the possibility to use a wide variety of patient’s specimens.
The aim of this study is to compare the sensitivity of the polimerase chain reaction after two different types of DNA extraction from sputa, tracheal and bronchial secrets, artificially infected with C. pneumoniae.

HEMODIAPERFUSION IN LEPTOSPIROSIS

ABSTRACT: Comparatively have been studied laboratory parameters in severe cases of leptospirosis divided in two groups – with and without hemodiaperfusion (HD). The dynamical investigations of blood urea nitrogen (BUN), serum creatinine, and serum bilirubin levels reveal that hemodiaperfusion markedly decreases the levels of nitrogen parameters and is non effective for serum bilirubin. Hemodiaperfusion in oliguric phase of the acute renal failure of leptospirosis prevents brain edema and lung edema - significant tanatogenic factors.

PANCREATIC INVOLVEMENT IN LEPTOSPIROSIS

ABSTRACT: The research includes 84 patients with leptospirosis treated in Clinic at Infectious Diseases – Pleven. Mild course in 52,38%, moderate in 19,05%, and severe in 28,57% are established. Serum amylase level has been investigated in 37 cases. Increased rates are measured in 41,67% of mild cases (av. 279 U/L), in 25% of moderate cases (av. 290 U/L), and in 88,24% of severe cases(av. 937 U/L). Significant difference between serum amylase levels of mild and severe cases is established (p < 0,02). Presumably immunological mechanisms of damage by participation of interleukins (IL1, IL6, IL8) and tumor necrosis factor (TNF-á) are in the base of increased serum amylase level in leptospirosis. The functional changes of pancreas in leptospirosis are not fully clear and further research is needed.

TANATOGENESIS IN LEPTOSPIROSIS

ABSTRACT: During the period 1982-2004 in our clinic 84 patients with leptospirosis have been treated. This study dynamically compares changes of laboratory parameters in survivors and fatal cases, and reveals following post mortem founded tanatogenic factors: brain edema in 100% of non survivors, lung edema (100%), severe cardiovascular disorders with shock (100%), and hemorrhagic syndrome (80%).

RECURRENT ANAPLASTIC MENINGIOMA AFTER SURGERY AND RADIATION THERAPY FOR BASAL CELL CARCINOMA - CASE REPORT

ABSTRACT: A case-report of 63-year female who developed meningioma after surgery and contact cranial radiotherapy for basal cell carcinoma (BCC) was presented. Initial symptoms appeared in a latency period of 14 years and included moderate frontal and occipital headache, nausea, speech disorder, gait instability, memory impairment and secondary generalized seizure. The tumor lesion visualized on the computed tomography (CT) and the medical history of previous cancer disease led initially our thinking to the diagnosis of cerebral metastasis. Histological verification confirmed anaplastic meningioma. Long-term follow-up of clinical, CT and electrophysiological findings corresponded with the recurrent course of the disease 10 months after gross-total tumor extirpation. Stable course of the disease was established since second operation until now.
Review of the literature concerning incidence and differential diagnosis of post-radiation brain tumors was done. The prior cranial irradiation as a possible risk factor for meningioma development was discussed.

AN ORIGINAL PC PROGRAM FOR REGISTRATION, STORAGE AND ANALYSIS OF DATABASE ABOUT PATIENTS WITH BRAIN TUMORS

ABSTRACT: An original PC program developed and designed for registration, storage and analysis of database about patients with primary and secondary brain tumors was presented. The so called “Program for operation of hospital charts of patients with brain neoplasms” was based on information from routine and modern clinical, instrumental and immunological methods for diagnosis, monitoring and prognosis. It was designed as a PC model of a chart for registration with unique mode of operation of patients’ entry characteristics, forming of their results into database and opportunity for calculation and prognosis of exit parameters such as probable survival rate and etc.
The main task of the program was to provide a useful data for the investigated patients with brain tumors. It was achieved by application of a real chart for collection and extraction of filtrated information about clinical cases with similar characteristics. The entire operation of the database was set up by Microsoft OLEDBDC driver 2003. The integral functionality concerning screen imaging, entry, correction, erasure, search and calculation of data was realized with program language Microsoft Visual C# 2003. The presented program was highly informative and easily available for routine administration in the neuro-oncological practice.

OLFACTORY NEUROBLASTOMA: CLINICAL PRESENTATION AND MANAGEMENT

ABSTRACT: Olfactory neuroblastoma (ONB) is a rare tumor. There is greater agreement for separating “neuroblastoma-like” forms from tumors arising in the same location and manifest morphological and immunohistochemical characteristics of neuroendocrine carcinoma.
The aim of present study is to describe the typical clinical, histological and immunohistochemical findings of ONB in 58-year old woman which help us to exclude non-Hodgkin’s lymphoma and endocrine carcinoma.

WORKING TIME ORGANIZATION AND MYOCARDIAL INFARCTION (First communication)

SUMMARY: The purpose of the investigation is to analyze by means of hospital based case-control study the influence of working time duration in acute myocardial infarction (AMI) patients. The results demonstrate that males prevail among AMI patients in any age groups except for the group over 76 years while at the working age, 60,9% of the subjects examined are mainly physical workers (Mann-Whitney U 1633, As. Sig. 0,02). There is a statistically significant overtime workday and workweek immediately prior to hospitalization as well as a systemic pattern concerning the length of service in the group with AMI in comparison with that in the control group. Wrong working time organization with overtime workday and workweek is a correctable risk factor for a healthy status.
