AESTHETIC RECONSTRUCTIONS OF I AND II CLASS CAVITIES BY SELF-ETCH ADPER PROMPT L-POP AND UNIVERSAL NANOCOMPOSITE FILTEK SUPREME IN AN UNDERGRADUATE COURSE

Abstract:
Adhesive systems and resin composite materials have developed dynamically. These materials have applied for an aesthetic reconstruction in posterior teeth and this tendency has increased significantly over the last several years. Student’s education is closely connected with learning and training in clinical situation new technologies’ materials.
The purpose of this study was to investigate practical students skills needed in aesthetic reconstructions of I and II class cavities by self- etch Adper Prompt L-Pop Adhesive™ and universal nanocomposite Filtek Supråme™.
A clinical follow- up was conducted on 37 resins composite restorations made in an undergraduate course over a 3-month period. It was investigated proximal contact reconstruction, condensable resin composite abilities and student’s efforts to obey clinical sequences in adhesive system application according manufacturers.
The authors concluded that technically less- sensitive materials would be suitable as the materials for undergraduate teaching programs where less- experienced students are involved in the restoration work. Such clinical follow- ups will help improve the teaching program and also provide valuable insight into performance of the materials used.

A SURVEY OF LOCAL ANESTHETIC AGENTS USED BY BULGARIAN DENTAL PRACTITIONERS

Abstract:
We examine what anesthetic agents, from those currently available for this purpose, are preferred in dental practice and are used in patients with arterial hypertension. Data were gathered by questionnaires which required information about the age, sex, specialty and place of work of dental practitioners; it also included questions about blood pressure measurement prior to dental treatment and subsequent selection of the anesthetic agent to be used according to the obtained value. The most preferred anesthetic agent was found to be ubistesine. Most of the respondents (77.73%) tend to take the patient’s blood pressure if there is a history of hypertension and the most frequently used anesthetic in such cases was lidocaine.

ABSTRACT:
Most of the endodontically treated teeth show a significant loss of dental tissue. The aim of this paper is to study the usage of different types of posts in incisors, premolars and molars of upper and lower jaw. The passive industrial posts are the most preferred and the most frequently used in the restoration of all groups of endodontically treated teeth.

CLINICAL TEST OF "DEOMINT" TOOTHPASTE

ABSTRACT
Toothpaste is the main means of oral hygiene. It contains active components that help achieving prophylactics of caries and parodontic diseases.
The purpose of this test is to study the therapeutic and prophylactic effect of "Deomint" toothpaste, which contains sorbitol, sodium monofluorphosphate, PEG - 400, hydroxyethilcellulose, aerosil, calcium carbonate and silica.
There have been reported PLI under Green & Wermillion, PMA index of Massler & Schour and IKS.

CLINICAL AND GENEALOGICAL STUDY ON A FAMILY PREDISPOSED TO PATHOLOGICAL TEETH ERUPTION.

We studied the members of a family with teeth retention and anodontia. The study included nine maternally related subjects. There were five women and four men aged between 15 and 60 years. The genetic examination showed the inheritance pattern to be of autosomal dominant type.

Application of the electroacupuncture analgesia (EAA) for surgical treatment of allergic predisposed patients

ABSTRACT:
The surgical intervention needs sufficient analgesia. The authors share their experience with on electroacupuncture (EAA) method in surgical treatment of allergic predisposed patients (man and women, age 25 – 60). The authors influence over 3 facial located acupuncture points and 2 distal bodies- located with OZ-22 (EAA-apparatus) for 20 – 25 min before the surgical intervention. The anesthesia rate, reactions of patients and the major vital parameters are observed, measured and evaluated with special score. Total or partial analgesia was found in 55% (10 patients).
The main characteristics of the EAA are low price, easy management, nontoxic and noninvasive technique. The authors considered EAA as a method of choice for patients with medication allergies and somatic chronic diseases.

ELECTROACUPUNCTURE ANALGESIA IN ORAL SURGERY

ABSTRACT:
The authors describe the major moments of the acupuncture method history and development. The classical “Chinese” period and the modern divisions of acupuncture as well as electroacupuncture analgesia (EAA) are pressured. The EAA application in oral surgery is point of interest for the authors. In the present review the efficiency of EAA during surgical treatment is discussed. It is harmless and easy applied technique, nontoxic and especially recommended for allergy predisposed patients.

INTRALIGAMENTARY ANESTHESIA IN GENERAL DENTAL PRACTICE

ABSTRACT:
The aim of the present study was to evaluate intraligamentary anesthesia as it is used in general dental practice. Material and methods: The study included 220 dental practitioners throughout Bulgaria who were interviewed anonymously using a direct group questionnaire survey. We analysed problems primarily related to the application, analgesic effect and complications of the anesthesia. The results of the study have shown that 75.91% of Bulgarian dental practitioners use intraligamentary anesthesia in almost all types of dental treatment. Complications were found to occur in 27.54%. The anesthesia was sufficiently effective only in 32.94%. Conclusion: Intraligamentary anesthesia is used frequently by Bulgarian dentists. The effect of this type of anesthesia is not always sufficient to induce effective analgesia and very often additional anesthetic procedures are necessary.

ASSESSMENT OF THE GENERAL HEALTH STATUS OF DENTAL PATIENTS BEFORE TEETH EXTRACTION

Abstract
The aim of the present study was to evaluate the general health status of patients that are planned to have teeth extraction under local anesthesia. Material and methods: We recruited 635 patients of those treated in the dental surgeries of the Department of Oral Surgery in the Faculty of Dentistry, Medical University, Plovdiv. Prospective questionnaire survey was used to assess their general health status. The questionnaire included demographic questions and questions about current disease. All concomitant diseases that could affect the application of local anesthetic agents were grouped into cardiovascular diseases, endocrine diseases, allergic diseases, and other diseases. Results: Most of the respondents were over 50 years old (66.33%). 43.94% of the patients reported presence of some concomitant disease, hypertension being the most prevalent disorder (34.49%). This disease poses a serious risk for patients who receive local anesthesia.

Preview: The purpose of the following resume is to introduce the effects of using platelet-rich plasma for boosting the bone formation when added to various bone, and bone replacing materials. It also deals with the characteristics of the platelet-rich plasma. Further, a review of many articles dealing with the above-mentioned issue is presented, with the conclusion that adding platelet-rich plasma in bone and bone replacing materials can significantly improve the healing process of bone and gum tissue in the surgically treated area.

PAIN, STRESS, ANXIETY AND PSYCHOTHERAPEUTIC MODALITIES FOR THEIR MANAGEMENT IN DENTAL PRACTICE

ABSTRACT: We reviewed 21 sources in the literature and made an exhaustive analysis of the stress-related anxiety states generated by pain during dental treatment. Different psychological and psychotherapeutic modalities for their management were discussed. The results of the study suggest that dental practitioners must know and use such modalities, and, if unable to deal with anxiety states, require consultation with psychotherapist.

ANTIBACTERIAL ACTIVITY OF INTRACANAL MEDICAMENTS AGAINST BACTERIAL ISOLATES IN CASES OF ACUTE PERIAPICAL PERIODONTITIS (NONEXUDATIVE FORM)

SUMMARY: The presence of microorganisms and derivative toxins in the infected root canals determine the changes that occur in the apical periodontium.
The aim in this in vitro study has been to evaluate the antimicrobial activity of the intracanal medicaments against bacterial isolates in cases with acute periapical periodontitis.
The following clinical isolates from cases with acute periodontitis (nonexudative form) were used in the present study: Streptococcus pyogenes, Streptococcus mitis, Streptococcus bovis, Streptococcus anginosus, Enterococcus faecalis, Staphylococcus aureus. We tested their sensitivity to some of the most frequently used intracanal medicaments (in vitro), i.e.: Calcium hydroxide paste (Calcident 450® W/P Dental, Germany), Sodium hypochlorite 3% (Switzerland), Metronidazole 0.5% (Troya Pharm, Bulgaria), Chlorhexidine gluconate 0.1% (Troya Pharm, Bulgaria), and a combination of Sodium hypochlorite 3% and Metronidazole 0.5%.
The results show that Calcium hydroxide had the strongest impact in direct contact with the studied microorganisms, followed by Chlorhexidine gluconate 0.1% and Sodium hypochlorite 3%. The combination of Sodium hypochlorite 3% + Metronidazole 0.5% is more active than Metronidazole 0.5% used on its own.

INDIVIDUAL TRANSFERRING OF CLINICAL AND PARACLINICAL DATA BY TOTAL REHABILITATION OF PATIENTS WITH COMPROMISED STABILITY OF THE FULL DENTURES. A pilot study

SUMMARY: Denture stability is of primary importance for the successful result of the prosthetic treatment of totally edentulous patients. The authors aim was to present their methods for selecting posterior artificial teeth form by patients with compromised stability of the full dentures. After a profile teleroentgenography and measuring the condylar pathways and its angle to the Camper’s line, different sets of posterior tooth form of artificial teeth were chosen- Orthocal, Artic (20? ) or Basic (30? ), Heraeus Kulzer, Gmbh&Co.KG. The new prosthetic treatment showed very good stability of the full dentures at function assessed by the patients themselves and checked by the clinical tests.

CRITICAL OBSERVATION OF THE ANATOMIC CAST MODELS BY PATIENTS WITH FULL DENTURES

ABSTRACT: Introduction. The maxillary and the mandibular anatomic casts of full dentures impressions should include all the anatomic surfaces of the anatomic impression as an appropriate individual tray to be performed.
The purpose of the authors is to present the most often mistakes by anatomic casts of full dentures’ first impressions made in the Prosthodontics’ exercises in the clinics of Prosthodontics, Faculty of Stomatology-Sofia by the students.
Materials and methods.On critical observation were subjected 48 anatomic casts (26 upper and 22 lower casts) of 28 patients (16 women and 12 men) with a forthcoming full dentures’ prosthodontic treatment. The results show that 37, 5% of the anatomic casts didn’t meet the requirements of an appropriate stone cast.
Discussion.The observed shortcomings and defects by the false casts draw our attention on the necessity of more detailed and well grounded explanation of oral anatomy in connection with the future role of the anatomic stone casts for the retention and stability and construction of the full dentures.

SELECTIVE PRESSURE IMPRESSIONS METHODS FOR TOTAL DENTURES BY PATIENTS WITH LOOSE AND HYPERMOBILE MUCOSA ON THE ALVEOLAR RIDGES

ABSTRACT: Introduction. The mucosa over the alveolar ridges by totally edentulous patients is with varying thickness and mobility at different places and is distorted at the time of impression procedure. This distortion duplicated in the finished dentures causes inflammation and instability of the dentures.
The aim of the authors is to present three methods for selective pressure impressions for total dentures’ treatment by patients who had different kind of localization and abnormalities of the thickness and mobility of the alveolar ridges’ mucosa.
Materials and methods .15 edentulous patients, 9 women and 6 men, mean age 64 years, with different kind of mucosal abnormalities were divided into three groups and treated with full dentures. Three different impression techniques are used for selective pressure impressions.
Results. The silicone pressure tests at the delivery appointment showed even layer of the material over the base plate of the dentures, except for two patients from the first group. with The patients mention for comfort and satisfaction during the usage of the new dentures, compared with the previous one.
Discussion. The secondary impression causes some displacement no matter how carefully is made. The suggested three methods eliminates the excessive displacement of the soft tissues at the secondary impression thus a physiologic and anatomic registration of the attached and the unattached tissue of the denture bearing areas are attained.

TOTAL REHABILITATION BY EDENTULOUS PATIENTS WITH IRREGULARITY OF THE ALVEOLAR RIDGES

ABSTRACT: Introduction. The treatment of edentulous patients is always a challenge for the prosthodontists. It is connected with a lot of procedures aiming to reconstruct and replace the lost natural dentition and associated structures of the mandible and maxilla for patients who have lost all their remaining teeth. The basic objectives of complete denture Prosthodontics are the restoration of function, facial appearance and the maintenance of patient’s health.
The aim of this work is the prosthetic treatment of patients with asymmetry of alveolar ridges, the difficulties connected with that treatment and the suitable techniques and approaches to overcome those problems.
Material and methods. Twenty patients, twelve women and eight men, with an average age of 64±7 years were included in this investigation. All of them had different type and degree of irregularity of the alveolar ridges. The complaints of the patients were mainly connected with poor denture stability during mastication and with the esthetics. The treatment strategy of those patients included excellent impression of the peripheral seal, very careful measurement of the parameters of the vertical and horizontal occlusal plane, lingualized occlusion and esthetics improvement procedures.
Results: All the twenty patients accepted the new dentures with comfort and esthetical satisfaction and assessed them to be significantly more satisfying in esthetic appearance, maxillary and mandibular denture retention and chewing ability. To six of them preliminary contacts were eliminated at the delivery appointment. On the second appointment by 3 patients from the third group and one from the first group decubital ulcers were treated.
Conclusion. As complete dentures are the last consideration for the patient (excluding implant treatment) they must be designed and constructed with an emphasis on the preservation of the remaining structures.
