CLASSIFICATIONS OF MANDIBULAR
FRACTURES-REVIEW
Hr. Mihailova
Department of Maxillo-facial radiology and oral diagnostic, Faculty
of Stomatology, Medical University-Sofia
ABSTRACT: The author makes a brief review of the most
familiar classifications of mandibular fractures. It shows that the base
of these classifications are the categories connected with anatomical
localization of the fractures.
It is point out the necessity of a classification which includes very
well defined categories. These categories should be visualized radiologically
and should be used easily by doctors with different specialties. The classification
of mandibular fractures should be handy for the daily work in Emergency
Departments.

CYSTIC LESION OF THE MAXILLA - CASE REPORT
Hr. Mihailova, Br. Kirilov*
Department of Maxillo-facial radiology and oral diagnostic, *Department
of Maxillo-facial surgery, Faculty of Stomatology, Medical University-Sofia
ABSTRACT: The authors submit a theme concerning a radiolucent
lesion of the maxilla. They emphasize on the fact that the clinical and
radiological features are difficult to differentiate extensive cyst lesion
from those of other odontogenic or non odontogenic benign tumor arising
in the upper jaw. The correct diagnose is very difficult and is possible
only after histological examination.
They present a case with a large cystic lesion of the maxilla hystologically
proved as keratocyst.
Key words: keratocyst, radiolucency, maxilla

SUMMARY: The use of contemporary composite materials
for shaping and restoring of the teeth combined with orthodontic treatment
gives great possibilities for dental treatment.
The aim is to present authors’ clinical experience of esthetic reconstruction
of distances between the teeth after orthodontic treatment with brackets,
by correcting the form and size of these teeth.
Application of direct composite restorations (light-curing hybrid material
Gradia Direct Anterior, GC) according suitably chosen four clinical cases
after orthodontic treatment, has been demonstrated. Color photos were
taken before and after reconstruction of the tooth form and outlines.
Closing distances between teeth by enlarging tooth size is suitable only
when an esthetic shape and size of the reconstructed teeth is expected.
This way of reconstruction has the following advantages: the esthetic
results are excellent, it preserves the Class I ratio of the canines and
particular height and inclination of the upper incisors in treatment with
premolar extraction only in maxilla and prevents prosthetic treatment
in the anterior teeth.

OUR EXPERIENCE WITH GRADIA DIRECT IN THE RESTORATION OF ANTERIOR
TEETH
Sn. Topalova-Pirinska, R. Pirinska
Department of conservative dentistry, Faculty of Stomatology, Medical
University, Sofia
SUMMARY: One of the greatest challenges in restorative
dentistry is creation of well balanced harmony of the natural teeth.
The aim is reconstruction of the integrity and esthetics of the anterior
teeth with Gradia Direct Anterior, GS.
Reconstructive treatment of dental erosions, cervical caries and restricted
fracture of the incisor edges of permanent anterior teeth was made using
composite material Gradia Direct Anterior, GC and multi shade layering
technique or just single shade technique. The patients drink Coca-Cola
and fruit juices too much. Colour photos before and after restoration
were made.
Often the pathological changes in the visible tooth structures cause esthetic,
psychological and social discomfort and the appearance of the teeth must
be improved. By balanced scientific achievements and improvement of the
practical precision the smiles on the face of our patients will be returned.

Department of Prosthetic Dentistry – Medical University , Plovdiv.
SUMMARY: Various methods have been described for controlling
the thickness of soft denture lining materials. However the procedure
described in this article has certain advantages.
This study reveals the benefit of using a vacuum-formed spacer, because
it permits a long- term soft lining with uniform thickness for brand new,
old, complete or partial dentures.

SUMMARY: This article describes certain changes that
take place during hardening of several kinds of cements and their influence
upon vital teeth and the expected reactions of the patients during treatment.
It also reveals the probable reason for failure after fixing crowns and
bridges. This study shows that unexpectedly “old- fashioned“ cements reveal
to certain extent better properties than some of the “up-to-date“ ones.

Department of Prosthetic Dentistry, Medical University – Plovdiv.
SUMMARY: A technique for estemating in vitro the accuracy
of denture bases , using a low-viscosity impression material for determing
the space between the die and the dentures upon it was developed.
The measurements showed that relining denture bases with autopolymerizing
acrylic resin improved their adaptation to the ridges.
The in vivo study showed no statistically significant change in stability
after relining , but the trend was toward increased stability with the
relined dentures.

CONNECTION BETWEEN SOME JOINT DISEASES AND FOCI
OF ODONTOGENIC ORIGIN
E. Sarachev
Department of Oral Surgery, Faculty of Dentistry, Medical University
– Plovdiv
ABSTRACT: The author focuses on the connection between
chronic foci of odontogenic origin and their role in the occurrence of
non-specific infecto-allergic arthritis, for the time being of uncertain
etiology.
It is pointed out that a proper radical treatment of stomatogenic foci
is required, so that recurrence of arthritic diseases can be avoided.

STOMATITIS PROSTHETICA-A POLYETIOLOGIC DISORDER
Hr. Hadjieva, M. Dimova, S. Todorov
Department of Prosthetic Dentistry, Faculty of Stomatology, Medical
University - Sofia
ABSTRACT: Introduction. The prosthetic stomatitis is a
multietiological condition. The clinical manifestation could be the same
nevertheless what is the etiology and at the same time one and the same
reason could result in different clinical manifestation.
The purpose of this investigation is to establish the frequency of denture
stomatitis, its etiology, diagnosis and treatment.
Materials and methods. The authors have investigated 300 patients with
removable complete and partial dentures. The patients with denture stomatitis
were 54. All of these 54 patients were subjected to microbiologic tests
and tests for allergy to pink and clear polymethylmethacrylate. The quality
of the prosthetic treatment was checked: the extension of the borders,
the vertical occlusal distance, the occlusion and the dentures’ fir to
the underlying mucosa.
Results: From all 300 patients 54 (18%) were diagnosed with prosthetic
stomatitis. By 18 patients (6%) we established oral candidosis, and by12
patients( 4%) we observed positive allergic tests. By 18 patients( 6%)
the reason for the inflammation was due to trauma from the prostheses.
By 27 patients(9%) the denture and oral hygiene were unsatisfactory.
Summary: Denture stomatitis is not a rare condition. Its treatment will
be successful if only the etiologic factor or factors are established.
Key words: denture stomatitis, allergic stomatitis, trauma
from dentures, Candida associated denture stomatitis

TOTAL PROSTHETICS IN FUNCTION
M. Dimova, Hr. Hadjieva
Department of Prosthetic Dentistry, Faculty of Stomatology, Medical
University - Sofia
ABSTRACT: Introduction. Biologic, physioliogic and mechanical
principles need to be considered and carefully coordinated in the establishment
of the total denture design and occlusion.
The aim of the authors is to share their experience with total denture
treatment by patients, edentulous more than 6 years, using the method
of intraoral registration of the position of the lower jaw by Gerber and
arranging the artificial teeth by Korholz.
Material and methods. Fourteen patients, six men and eight women, mean
age 61 years, edentulous more than 6 years were treated. After all the
requirements of a functional impressions and determining of the vertical
jaw relation are kept, the central position of the lower jaw was fixed
by intraoral registration by Gerber, using the face bow "Kavo"
and transferring plates. The models were mounted into an articulator "Protar"
IV and the artificial teeth (Ivoclar-ortotype) were arranged by the method
of Korholz.
Results and discussion. The patients were asked about their experience
and comfort with the new dentures and subjective tests for estimation
of dentures' stability were applied. The quick and full adaptation to
the dentures and the scarce need of corrections allow us to recommend
this method of treatment by patients, edentulous from seven years and
with advanced atrophy of the jaws.
Conclusions: Using the intraoral graphic registration of the central position
is a substantial step towards denture accuracy and stability. The arrangement
of the teeth by Korholz is a prime factor to this goal.

CHANGES IN CORRELATIONS BETWEEN CERVICAL CROWN
EDGE AND MARGINAL GINGIVA IN FIXED PROSTHODONTICS
M. Dimova, V. Doseva
Department of Prosthetic Dentistry and Department of Periodontology,
Faculty of Stomatology, Medical University - Sofia
ABSTRACT: Gingival adaptation of fixed prosthodontics
and periodontal tissue’s assessment place the main role in prosthodontics
long term results.
The aim of this study was to assess changes in correlations between cervical
crown edge and marginal gingiva in patiens with fixed prosthodontics.
Material and methods: 54 patients: 31 women and 23 men, aged from 32 to
71 years (middle age- 52 years) were examined. Patients were with different
fixed constructions: full metal, metal-ceramic, acrylic and PFM crowns
and bridges with 2 to 25 years longevity (middle duration of presence-
14 years).
Clinical periodontal examinations included: hygiene index, gingival bleeding
(PBI), the distance between cervical crown edge and marginal gingiva,
probing pocket depth, clinical attachment level, gingival overgrowth,
tooth mobility.
Results: Clinical examination indicated little inflammation of the gingival
tissues of crowned teeth with margins at the gingival and minor clinical
signs of inflammation in cases with supragingivally location of the crown
margin. Higher average of PBI was observed when the crown margins were
located subgingivally. In summary- crowns and fixed prosthodontics increased
the incidence of advanced gingival inflammation adjacent to restorations,
particularly if they had intracrevicular finish line placement, poor marginal
adaptation, and rough surfaces.
Conclusion: The main factors to realize periodontal prophylactic role
of fixed prostodontics and to preserve periodontal health are exact adaptation
of crowns and retainers toward gingival preparation limits, maximum release
of the interdental spaces, correct contour of the crowns, good polishing,
motivation and instruction for mouth hygiene of the patient.
